Post 3, Menopause: Feeling Optimal Isn't a Luxury — It's the Point

We've covered a lot of ground in this series. In Part 1, we talked about the exhausting experience of being told your labs are "normal" while you feel like a stranger in your own body. In Part 2, we walked through why hormone therapy got sidelined for two decades, and how the science — and finally, the FDA — caught up in November 2025. So here's the question this last post is really about: Now What?

The Scenario I Never Get Tired Of

There's a moment I love in this work. A patient who came in exhausted, foggy, and half-convinced she just had to accept "this is 45 now" comes back a few months later and says something like, "I forgot what it felt like to want to get out of bed." That's not a hot flash going away. A 2025 study published in Neurology found that women who initiate estradiol-based hormone therapy closer to the onset of menopause show measurably different cognitive performance patterns than those who start later or not at all — reinforcing what researchers call a "critical window" for brain health. Separately, a 2023 analysis by neuroscientist Dr. Lisa Mosconi and her team, reviewing more than 50 studies, found that women using estrogen therapy within about 10 years of their final period showed a significantly lower risk of dementia, while starting combination therapy after 65 was linked to increased risk — timing, again, being the variable that changes everything.

Bone health tells a similar story. A 2026 cohort study presented through the Endocrine Society found that women using menopausal hormone therapy had roughly a 69% lower risk of low bone mineral density in the spine and hip compared to non-users, even after accounting for age, time since menopause, and vitamin D status.

This is the "so what" behind everything in this series: the choices you make in this transition don't just determine how you feel this year. They shape your brain health, your fracture risk, and your independence for decades to come.
